Patient Center (II) is the fourth chapter or section of the story in Resident Evil Requiem, and it will begin automatically after raising the gates in the western hallway of the Patient Center. As usual, throughout this walkthrough we will explain how to progress while completing as much of the game's content as possible, avoiding spoilers whenever possible while ensuring that you obtain all the collectibles, receive help with puzzles and much more.
Follow the attendant without bothering to explore, as you cannot interact with anything. A cutscene will begin after you cross the room filled with stretchers.
Evidently, Leon has no lingering trauma involving lunatics armed with potentially lethal gardening equipment. The game will teach you how to parry with the axe. Do so to block the doctor's attack and unlock the Untouchable achievement/trophy. If you get it wrong, there will be two Leons instead of one... well, something like that. If you manage to kill an infected enemy wielding a chainsaw before another picks it up, you can collect it and use it to attack an enemy.
Bear in mind that you cannot approach the chainsaw until it stops moving, and any infected enemy can pick it up. The movements of chainsaw-wielding enemies are unpredictable, fast and lethal. It is not a bad idea to retreat and wait for the first enemies to line up, allowing you to eliminate at least two with a single shot from the Requiem before finishing off the others with headshots. Then wait for the chainsaw to stop, pick it up and use it to eliminate the new enemies. Be careful, as they will approach from both the room at the far end and the stretcher area.
Once you have defeated them all, make sure to search the floor. The enemies should have dropped some resources, primarily ammunition. Do not forget to sharpen the axe. There is no limit to how many times you can do so, and it is wise to keep it ready for an unexpected parry or melee attack. There are also some items on a couple of glass-fronted shelves. Collect everything, pick up the chainsaw and head to the southern door, which is barred shut.
Continue along the only available route and open the door at the far end. To the left of the stairs is the first Mr. Raccoon. There are 25 in total, with several related achievements/trophies, so shoot it to "collect" it or strike it with the axe. You will unlock the The Hunt Begins achievement/trophy.
Now climb the stairs and follow the only available route until you reach the Central Hall. The gate on the right may look familiar. Approach it to see what happens. After a couple of awkward moments, we will return to the Patient Center with Grace.
