
Patient Center (V) is the seventh chapter or seventh part of the story of Resident Evil Requiem. It begins after using the elevator in Victor's office at the end of the previous section with Leon. As usual, throughout this walkthrough we will provide directions to help you complete as much of the game as possible, avoiding spoilers wherever possible while ensuring that you obtain all collectibles, receive help with puzzles and more.
Continue through the tunnel. It is not as though you can do much else. After taking a few steps, you will find a body and the Staff Members' Last Words document. You will pass several bodies that will obviously get up later, but they are not a threat for now. Continue until you see an open door on your right.
On a table, you will find the Operation to Eliminate "the Girl" document. You are in a safe room, so reorganize your inventory, collect the Ink Ribbon Packs and return to the tunnel. Follow the only available path until a cutscene begins.
Continue east and examine the panel above the switches to obtain the Opening the Cells document.
Start by removing the Junction Plug from the machine beside the lever. You can now open the areas that were locked as you made your way here. First, head to the northwestern exit from the Cells... but the Girl will appear at the far end of the hallway.
If you hide around the corner beside the lever, you can wait for her to take a look at Emily and leave. Now head to the northwestern corner of the Cells and use the plug to enter the Workshop.
Inside, you will find a Mr. Raccoon and the Technician's Note document. Leave the Workshop, retrieve the plug and head to the southwestern door leading to the Furnace, then open it with the Junction Plug.
Eliminate the zombie, collect some resources and enter the office in the northern section.
Inside, you will find the Security Guard's Report document and a safe (60-40-80). You will obtain handgun ammunition and a Mr. Raccoon Map (Rhodes Hill), although you will not need it. That is why you have Captain Obvious holding your hand.
Now look at the forklift and the plug beside it.
Collect the plug and enter the tunnel north of the forklift, crouching to pass through it. You will find a Lock Pick, although there is currently nothing left to open with it. Continue to the end without shooting the zombie. You will emerge in the Workshop. If you did not notice it earlier, there is a crate you can climb in one corner, allowing you to leave through an open window.
Head to the Furnace and do not remove the Junction Plug. A zombie will get up there and the Girl will approach from behind you, but she cannot enter the illuminated area. Return to the area with the Item Box and take out the Requiem with at least one round. We are going to head to the northeastern corner of the tunnels, where you can insert the Junction Plug and open the door to a small storage room. Push the crate at the far end to enter the Boiler area.
On the floor near the eastern wall, you will find another Lock Pick.
Now head to the western area and collect the Square-End Key from the toolbox. When you try to leave, the Girl will appear. Use the Requiem and shoot her once to unlock the Heavy Artillery achievement/trophy, slip through the bars and wait for her to leave. Remove the plug and insert it into the distributor in the southern hallway.
This will open the door to the room immediately south, which contains two zombies. Eliminate them and search the room to find ammunition, instructions for crafting Molotov Cocktails—meaning you can finally put those Empty Bottles to much better use—a Mr. Raccoon on top of the television and various resources. There is a lock in the southeastern corner that you can pick. It contains Rare Metal.
Before collecting the plug, head west in case the Girl appears. Wait for her to leave, then remove the Junction Plug from the distributor in front of the room. Head towards the Processing Tank.
As you descend the stairs, you will see some Scrap beside them. Circle the hole and you will find a machine on which you can use the Square-End Key. You will see that it requires two Junction Plugs. Remove the one from the Furnace door and use it to activate the machine. Do it quickly: the zombie beside the machine will get up and the Girl will appear, but she will flee once you have connected both plugs.
Continue southwest and pass through the fan directly ahead, as the path will collapse when you try to cross it.
There is Scrap inside the tunnel, and when you emerge you will find a Green Herb beside the ladder you need to climb. This will take you to the Processing area. Continue to the far end of the walkway and collect the Forklift Key beside the corpse. Use it on the nearby machine to lift the crates. This will allow you to enter the adjacent room, where you will find various resources and, more importantly, a new Hip Pouch.
You will also find the Patient Center Basement Map and instructions explaining how to activate the Processing area. For now, you must backtrack without taking the plug. When you reach the Processing Tank, it is advisable to use a Hemolytic Injector on the zombie to prevent it from transforming into a Mutant Head. You will pass through this area several more times, and you do not want any nasty surprises. Remove both plugs and head to the Forklift in the northwest.
Be extremely careful, as the Girl may appear here. If you are quick, you can reach the electrical distributor beside the Forklift and insert the plug to frighten her away. You can now activate the Forklift. As soon as you cross, look to the left to see two lockers. You will need to pick the lock on one of them. Once again, you will obtain Rare Metal.
Now look at the bodies rotating on the hooks. Make sure that you move the lever to the right—if you have not moved it, it should already be pointing in that direction—and look for a gap you can slip through, turning left as soon as possible.
You will reach a space in the centre containing a zombie, some items and a crate that you can move to open a path towards the lever.
Go through and move the lever to the left to reverse the direction in which the bodies are moving. Cross through the shortcut and note that you can now slip through a gap to continue south.
Another zombie will appear, and there are a couple of items you can collect—check the southeastern corner—before you continue south along the walkway. Turn left as soon as possible and proceed to the end to find some items and a typewriter. You should use it before turning the crank. Look towards the southeastern side of the pit to see a ladder. That is where you need to go.
The problem is that this is obviously a trap. As soon as you drop down, collect the resources from the walls and run away from the machine. If a zombie gets up, a single shot to the knee will be enough to let you shove it in this area. Try to make it strike several others as it falls, taking them down with it. Once the machine finally stops, climb the ladder and collect the Junction Plug. Backtrack and pass through the fan beside the typewriter.
After leaving the pit, look for a circuit breaker on one side to drain the pool. This will allow you to collect blood with the Extractor if you wish. Afterwards, use the elevator to return to the main area. You should take out the Requiem, as you may need it.
At some point, a Mutant Head will appear. It may be the zombie from the Furnace or the one beside the Forklift. Either way, you must retrieve both Junction Plugs—the one beside the Forklift and the one in the Furnace door—and take them to the distributor on the eastern side of the hallway beside the Cells. The Girl may appear whenever you remove one of the plugs. Each time you disconnect one, move away slightly in case she appears, then quickly put it back to frighten her off.
Do not waste a single second and run west along the hallway. Your objective is to reach the elevator in the southeastern corner. Do not enter the cells to search them. It is not worth it, and you will be killed.
Once you put Emily down, do not attempt to return to the cells. Quickly move the sandbags. When the elevator stops, open the doors. Once the sequence ends, you will obtain the Star Quartz.
First, collect the Severed Hand beside the elevator. We are going to take one final detour. Leave Emily in the Guard's Office and take out all the Ink Ribbon Packs and Empty Bottles you have. Now search for every source of blood throughout the Patient Center and use it to craft Ink Ribbons, Molotov Cocktails, Injections, ammunition... whatever you can. The reason for doing this is that there is an achievement/trophy for collecting 5,000 units of blood. This will add another few hundred to your total.
Once you are finished, pick up the girl and the Star Quartz, then open the northern door in the Central Hall.
Head north. There is nothing keeping you here. For some reason, there is an Improvised Knife in the Courtyard pond, but that is all... so take the western exit.
Just when I thought I was out... they pull me back in. Enter the damn house. I am sure it is not a trap or anything. As soon as you enter, collect the Visitor Log from the cabinet directly ahead, where the bottles are kept.
Continue forward and you will see an Item Box on the right. I am not paranoid, but this is starting to smell increasingly suspicious. Enter the next room and examine the bottle on the left. You must rotate it and align the label to complete the image. You can then separate the fake bottle to obtain the Double Helix Model. Now look at the desk at the far end and interact with the hourglass. Examine it and remove the screws from the sides to obtain... well, an Hourglass.
You can also find the Study Results Note document on the desk. Now examine the lectern and place the Double Helix Model. In the next room, between the Item Box and the typewriter, you will find the Spencer Research II document.
Now head down the stairs to reach a seriously nasty area. You should find an Injection inside a drawer to the northwest. Also note the waste canisters on the walls, as they may prove useful later. Head northeast and you will find the Subject 170 Log document on a stretcher.
You will find the Laboratory Map on the wall to the left. Collect it, then climb the stairs until you reach the Private Laboratory area, beyond the Development Room. As soon as you enter, check the northeastern corner of the room to find another document, the Grace Ashcroft Security Report, along with an Ink Ribbon. Now enter the laboratory and shoot the Mr. Raccoon on the table.
Now collect the Helicopter Key in front of the computer. Interact with the screen to unlock the Control Room door and trigger a cutscene. Once it ends, open the door through which the man left and head back downstairs. This time, you can pick up the waste canisters and use them to spray and blow up the zombies.
Go through the northeastern door and, after the cutscene, RUN STRAIGHT AHEAD AND THEN TURN LEFT.
It is technically possible to evade every enemy, although the Requiem may come in handy for putting a couple of rounds into the Mutant Heads if you want to get through safely. If you start running as soon as the sequence in which the creature drops from the ceiling ends, you can climb the stairs and turn right farther ahead when you see a Mutant Head getting up at the far end of the tunnel.
After climbing the ladder, head down the slope and enter the house through the window, ignoring the zombies. You can make any necessary inventory changes before heading towards the helicopter. Once the cutscene ends, you will switch to lending a hand as Leon.
